Congratulations on your pregnancy
This is a very exciting time and there is no doubt that you will have many questions that need to be answered now that your pregnancy has been confirmed.
Under a new scheme from Musgrove Park Hospital, they request that all pregnant women complete a maternity online self-referral form via their website. Please complete the Self Referral form.
Once you have completed the online form this will enable the midwife to contact you to make your 1st appointment. Important information which you will need to read before your 1st appointment with the midwife.
