A fit note must be issued by a healthcare professional, which could be a doctor, nurse, pharmacist, physiotherapy, consultant or occupational therapist. You are not always required to be seen in person to request one. This depends on;
- Why you are off work sick
- If a healthcare professional needs to assess you face to face
- If you have been in hospital
"Fit note" is the informal name for a Statement of Fitness for Work, also known as medical statements (MED3).
When do I need a fit note?
If you have been off of work for 7 days of less, you employer does not need medical evidence that you have been unwell. Instead they may require you to confirm this, by completing a self-certificate online. Visit GOV.UK website to complete one.
Should you be off of work for more than 7 days sick, your employer will usually ask you for a fit note from your GP or other health care professional. To request one from your GP please visit app.herohealth.net/nhs/523/care-navigation to complete an online consultation form.
